What is a shift report?

A shift report is a short written record left by the outgoing employee for the person taking over, covering what changed during the shift and what still needs attention. It goes by several names depending on the sector: shift handover, pass-down report, handover notes, or shift log. The purpose is narrow on purpose. It tells the next person what to act on, not everything that happened.

What should a shift report include?

Five sections cover almost every business:

  • Carry-over: what happened this shift that the next person has to do something about.
  • Open loops: anything unresolved, each with a named owner and a deadline.
  • Coverage: who was in, who was late, who didn’t show, what was left uncovered.
  • Equipment and stock: what’s broken, low, ordered, or waiting on a repair.
  • Customer or resident flags: anything needing follow-up next shift.

Add a header block with the date, shift times, location, and both names, then a signature line for the person taking over. This template includes all of it.

What should you leave out of a shift report?

Ask three questions before writing anything down.

  1. Does it matter for the next shift only? That’s a handover note, and it belongs here.
  2. Does it matter every shift? That’s a task, so it goes on the checklist attached to the shift.
  3. Does it matter to everyone? That’s an announcement, so it goes where the whole team sees it once.

One last test for whatever survives: if it will still be relevant two shifts from now, it isn’t a handover note either.

Who fills out a shift report and who signs it?

The employee finishing the shift fills it out. The employee starting the shift reads it and signs it, with the time and date. Both names appear on the page, which is what turns a note left on a counter into an actual handover. In larger teams, the supervisor or shift lead usually reviews the reports at the end of the week to spot recurring issues.

What’s the difference between a shift report and an incident report?

A shift report is routine and covers a whole shift, including small things that need follow-up. An incident report documents one specific event, usually something that caused or could have caused damage, injury, or illness, and it often has legal or insurance implications. If something serious happened, note it briefly in the shift report and file a proper incident report as well. One does not replace the other.

What does a shift report look like in different industries?

The five sections stay the same, but what fills them changes:

  • Retail: equipment issues, cash handling, held items awaiting pickup.
  • Restaurants: prep, equipment, large reservations coming in.
  • Residences and home care: resident condition changes, family contact requests, medication timing.
  • Security: incidents, access problems, patrol gaps.
  • Hotels: room status, guest requests, late arrivals.
